I have literally been looking for this fanfiction for years and I still can find it. I read it on fanfiction.net and it was completed.

The basic story line is that somehow (I can't remember exactly) Hermione is sent into the past, to Severus. She is magically sent to times in his life when he needs her the most, she doesn't stay in any for very long, just enough to help him get through. At the end she is sent back to her time where Snape was believed to have been killed in the battle but she finds out he's actually alive.

I don't know if that was a good descritpion or not but it's the best I can do. If anyone knows what it could be let me know.
