Was the "accident" planned? Was the crash deliberately caused? If so by who and for what reason? If the driver lost control of the car was it purely or partly due to him being intoxicated and/or driving dangerously? Could her or the lives of the other passengers have been saved if medical attention had been provided sooner?

Severe penalties are imposed by the Courts for drivers caught without insurance. Some might say rightly so. You are told in Court that you could have maimed or killed someone and be paying out for the rest of your life and have caused unnecessary distress and hardship to the family of your victim all because you did not have insurance.
Unfortunately in the UK insurance is issued by private companies that often mislead you and confuse you with facts and figures and technical jargon. You end up paying 5 times as much as your Bulgarian neighbour who has the same sort of car as you but who is lucky enough to have his car registered, taxed and insured in Bulgaria by the Bulgarian government that allows him to drive for 6 months in the UK under an international treaty.
You have no choice but to pay up! But when it comes to making a claim your insurers suddenly bring to light all these get-out clauses. You are then told they won't pay you out because of this thing or that thing.
With direct government control you would get rid of corrupt profiteers. Also the police would be able to tell immediately who is insured for what without having to stop you and find out who your brokers or underwriters are what you are insured to drive. It will also stop wrongful seizures of vehicles because of "reasonable suspicion" of no insurance.

If so, what were they unhappy about with being in the Union and were their grievances satisfactorily addressed by the UK parliament at the time or since then? If not, why not?
