When you contact your state for proof of birth, they send you a document much like the one posted by the Obama Campaign, and later shown in photographs on Factcheck.org. My PA certification has even less info.
His name at birth, according to the official document from Hawaii, was Barack Hussein Obama II.

The President has refused to produce his Birth Certificate. We do not know if if he was born in Hawaii or what name appears on his Birth Certificate.
