How do you interpret a phrase like "right-wing nut jobs" or "dirty liberals"?

Would the speaker be referring to the admittedly smaller group of people who are both "right-wing" and "nut jobs" (resp. "dirty" and "liberals"), or would the speaker be referring to everyone "right-wing"/"liberal" and stating "nut job"/"dirty" is implied?

Like most things it should depend on the speaker, but most of the people who say these phrases are both ignorant and stubborn, usually using it to demean a group without having to substantively respond to claims by them usually due to not being able to. As such, to accept there is anyone legitimate in the group would force a coherent response meaning they must refer to all right-wing/liberals as it to ensure they do not have to respond.
