Question about Ramadan from a non-Muslim?

I didn't actually know till today when I was talking to the local shop-keeper that the fasting includes water... it made me wonder, what happens if you have to take medication? Would you then be allowed to drink water, to wash the tablet down with? (would you be allowed to take the medication at all, or would you have to wait till evening?)

Yes it includes all liquids and all foods.
However, if one is to sick and needs to take medication or such do not need to fast at all.
Those who breast feed, are pregnant, too young, too old, to ill, traveling, don't need to fast unless they want to. Its solely on them. However (pregnant and breast feeding shouldn't fast at all)
And those on there cycle can not fast until they cycle is over.

Even the medication can be taken only after the hour of iftar, so, that's not any issue, actually ... because the medicine taken after iftar may be washed down with water.

If one can't do that, then they don't start the fast.
