The Nielsen ratings use a sample audience and then they extrapolate the result across the country.

My sister is a Nielsen rater. She has a "set meter" on her tv that sends info to Nielsen on what she is watching. Nielsen takes into consideration her demographic, and then makes a judgement on how many similar demographics are watching the same show.

The same method is used in political polling. A handful of people are asked who they are voting for and the results are extrapolated.
