I am trying to get inducted in to the National society of leadership and success. I need to do some community service. I know how to log into my member's area and check my membership status. I think I am supposed to get a form to fill out when I do my community service, but I cannot find it. Can anyone tell me how to find it? Or how to go about this?

Log into the Members' Area, and at the top left you will see your advisor and student president's names as hyperlinks. Click on the president, and drop him/her an email about what kind of proof they want. Usually a note signed by the coordinator of the project is sufficient: there isn't currently a national form.
