National Society of Leadership and Success Community Service form?

I am trying to get inducted in to the National society of leadership and success. I need to do some community service. I know how to log into my member's area and check my membership status. I think I am supposed to get a form to fill out when I do my community service, but I cannot find it. Can anyone tell me how to find it? Or how to go about this?

    I suggest that you create a journal. You write down the date/time/place you volunteered. Have someone there sign your log. Then, when your time is done, take a few moments and write down what you learned. This is the important part. Colleges don't really want to know how many hours you volunteered, they want to know what you learned, how you grew, why it was important in your life. If you jot this down after each time you volunteer, you'll be ready for those applications. (and you will be able to reflect how important volunteering is in your life). Hope this helps.
