My doctor told me work is not exercise! Is work exercise if you are doing heavy lifting, moving, and pulling carts ???

Of course heavy lifting is exercise. But your doctor is referring to cardiovascular exercise - getting your heart rate up for half an hour or whatever is the recommended time these days. Speed walking, jogging, cycling, that kind of thing is what he's talking about.

If you're a cart man pulling and pushing carts all day moving and lifting boxes and putting them on and off of trucks etc etc you're doing plenty of exercise.

20 minutes straight without a break and stretching and an over all conditioning routine with
Moderate weight lifting to stay tone and in
Good over all shape. Think in terms of what
Muscle and joint and core and over all body
Parts need to stay flexible and firm so they
Work fluidly and you can move without problems. Most people never build their forearms
By using pull up bars or rock climbing. So
They can't even hold their own weight for
Over a minute if the try to climb a wall of
Tall tree. So try to keep everything strong
Enough to support your weight and help
You survive or move quickly if you need to.

Yes, of course it is. 

He may be referring to aerobic exercise, the kind that gets your heart and breathing rate up for a sustained period of time and promotes the circulation of oxygen through your blood. That's pretty much considered the most desirable and healthy type of exercise.

IF your heart rate goes up, and you sweat.  I would say so.
