I am pretty sure I am only covered in my state by certain providers unless I go over my large deductible.

I guess since you said "generally," the answer would be yes.  Most PPOs typically use a national network.

It was standard pre ACA... since costs have to be contained somehow elimination of benefits is the only way to do it.  My PPO plan (Blue Shield of Calif) still has US coverage in every state but eliminated international coverage in 2019.
