In addition to the citation in Wikipedia, a couple of notes:
A former programmer for Digital Equipment told me that in the source code for one of their early operating systems where subroutine entry points labeled: Foo, Bar, and Foobar.
In the "ancient" computer game Adventure, in one rooom, there is a sign that says "Fie, Fi, Foe, Foo".
