Being french myself and speaking french fluently, I can answer your question:
- "Quel est votre nom?" "Comment vous appelez-vous?"
(polite form, if you ask this to one person);
- "Quel est ton nom?" "Comment t'appelles-tu?"
(if you ask this to one person and that you feel you don't have to use the polite form);
- "Quel est votre nom?" "Comment vous appelez-vous?"
(if you ask this to several persons, using the polite form or not).

Comment t'appelles-tu?
Comment est-ce que tu t'appelles?
Quel est ton prénom?
Is that enough? Each is right, but the first is less formal, it is not even grammatically correct, but it is how we say it in a casual way.
