how are jpg images actually stored on the disk?

I mean, what is the exact algorithm to create or modify a jpg image from scratch?

    The exact algorithm is very complicated. I believe it depends on dividing the image up into 8x8 squares, selecting a grid size based on the complexity of the image there, and applying a cosine transform.
