The more tone of the gastrocnemius and soleus muscles, and the less fat present, the more it will seem to pop out. More fat around the ankle would hide the tendon.

It means that Obamas policies and actions in Libya are his "Achilles Heel," being his one weekness. Also note that Romney has the arrow pointed at him, this indicates that he may be shooting himself by focusing on Libya.
