Send your manager a certified letter more than 30 days prior to your lease expiring. I've had a few business writing courses so I thought it would be fun to type you up a sample. I hope this helps!

Dear (Apartment Manager’s Name):
My lease on Apartment #__ expires on (date). According to section ___ in our lease agreement, I am to give you thirty days notice that I will not be renewing my lease. Please consider this letter to be notice that I will not be renewing my lease. I have enjoyed my stay here at (name of apartment complex). I will be completely moved out and will have finished cleaning the apartment by (the last day of your lease). I will call you a week prior to that time to make an appointment to return my keys, walk through the unit, and receive my security deposit.
If you have any questions or need to speak with me please call me at (123) 456-7890.

Yes, you just write in your own words when you plan to vacate. If you want to cover yourself, send it with a return receipt so you have a record of when they received it.
If it's a professional apartment manager, they probably have a form you just go in and sign.

BTW - why does it matter if the manager is "alert"?

Typically you would use either a pen and paper, or you can even write the letter on a typewriter or computer and print it on a printer. Here in the US we usually write from left to right top to bottom.
