    This means an assessment of all functions and problems. It would involve any or all of the following : Geriatrician, psychiatrist,physio, occupational therapist, district nurse, social worker etc.

    Multi disciplinary assessment means an assessment that can be done by more than one discipline. For example, a PT, ST, OT, or nurse might do the assessment. One of the best assessment tools out there for patients with dementia is the Mini Mental Examination. It is very helpful in determining just how demented the patient is. Hope this helps!

    It is a assessment made by a team of professionals. General Dr, Psychologist, Psychiatrist, Social Worker, Speech and Language, Occupational Therapist, Mental Health Team..

    Source(s): I work with autistic children who also have to have multi-disciplinary assessment so I'm not totally sure exactly who would be involved in a dementia sufferer but it will be a group of people with those titles or similar.

    It covers all the different assessments, mind, physical, emotional, and social aspects. Example, Physical Therapy, Dietary, Social/family aspects, loss of motor skills...etc. Also includes mental status, physical status, and if they use meds in the holistic realm as well.
