C12H22O11 is one molecule, you can tell as long as there is no number at the beginning (eg. 2 C12H22O11 means that there are two molecules of sugar)
Since there is one molecule, the numbers exactly after each letter constitues how much of that letter there is in the substance.

So there are 12 C, 22 H and 11 O
Add that up and we get 45 atoms.

If there were more than one molecule, we would multiple the answer above by the number infront of the formula.
