From Malpensa, there's a shuttle bus to Milano Centrale train station. It leaves from right in front of the terminal. You can buy train tickets from machines at the station or go to one of the ticket windows. The machinews are easy to use and have several language choices.
The fare for the train will be between 7.10 euro and 18 euro depending on the type of train and class of service you pick. There are several trains per hour. You can check times and prices on-line at: http://www.trenitalia.com/en/index.html
