It was a bald cap. He wanted the rest of the staff to do it too.

I'm pretty sure it was a bald cap because I don't think JD would want to shave his head for like, 1 episode.

He shaved it for his leukemia patient. He didn't want to at first, but he did it.

he actually shaved it cause the next episode he says is one month later, and his hair had grown back, and he also lost his girlfriend who was opsebbed with hair
