It means he's out of the game, and the DH is done for the day, and the pitcher has to bat.

The same thing that happens to any other player replaced by a pinch hitter: he's removed from the game.
Though, bringing in a PH for your DH would be rare and likely only occur as a result of injury.

Usually doesn't happen unless the guy gets hurt...but yes if the starting DH is pinched hit for..he's done for the game.

He's out of the game. It's the same for anyone in the batting order, once you're pulled from the game you're done for the day.
