The deceased is a fellow Lion and you want to send a sympathy card to his grieving family. Is that it?
Okay, on the card itself ---
IF you know the widow personally "Dear Julia.... Sincerely yours, GP (your name)
IF you don't know her personnaly "Dear Mrs. X.... I'm so sorry for your loss. John (the deceased) and I were fellow members of the Lions Club. He would be missed by all. ...Sincerely, GP (your name)
On the envelope -

I would call him by his first name or whatever you address him as normally. On the card you mail, I would address it to Mr. Bill Smith. If you add something handwritten on the inside just put: Dear Bill, I'm sorry for your loss...

No. Write to him as a friend, not a member. Thats what he needs most right now- friendship and sympathy.
