Does he like Rice Krispie Squares? I made a cake out of that for a friend who doesn't like sweets.
You could also make a hard meringue base topped with Strawberries and a little whipped Cream (Sponge cake could also be used)
If he will eat fresh fruit, make up dishes of homemade fruit salad and take something like a strawberry to stick the candle in and have stabalized in the dish by the other fruit.
Depending on what your meal would be, you could even stick a candle in any main meal food as well, so long as it is stabalized well.

What kind of food does he like? Are you looking to make a dessert "birthday cake" that isn't too sweet or chocolately? Or are you trying to stay away from sweet all together?
If you are trying to stay away from sweet all together, maybe you can make a casserole and put a candle in that. Or you could try a thick dip.

My fiance isn't wild about cake either. I frequently make him an angel food cake with fruit he likes. I bake 2 layers in square pans then ice it with stabilized whip cream icing. I put the fruit between the layers and write on the top of the cake and sometimes make a design with cut strawberries or raspberries. Its a lighter and less sweet cake and he loves it.

FRUIT TART
Made w/ pre-made puff pastry (comes frozen) Follow directions on package for oven temps
Cut your base crusts with clean can...cut another one same size but with a hole in the center to make your "edge-crust"...place on top 1st one with a little egg/water wash and use same eggwash to lightly brush top layer and center part (quickly scoring top crust with a sharp knife, not a fork gives it a pro look) and bake acc. to directions
That whole business takes 10-15 minutes! Cooling crust takes the time it takes to make filling
Make cream filling from scratch or use pudding mix but add 1/2 &1/2 instead of milk and maybe some fresh vanilla
Spread cream filling in tart shells..decorate with fresh or well drained frozen fruits
Glaze with either Orange marmalade or Raspberry Jam that you have slightly thinned & zapped
A candle and a sprig of fresh mint (optional, of course) and you've got a very pretty fresh dessert (easy too)
Note: pre-made puff pastry can be used a ton of ways from savory meat fillings to impressive layered dessert concoctions... lack of imagination is the only limit!

Make a birthday pizza! Or if you have biscuits with dinner use one of them. Hope that helps. Or how about cheese cake?
