You will need a D1 application form from the post office. They are not available online. You will need to put your new address in section one of the form. Fill in the rest of the form but no new photos will be needed and also no ID. Send it off with any part of the licence you may still have and a payment of £22. You would have to pay for it because you have lost both parts of the licence and we will need to issued a whole new one. Sorry about that. If you need any further advice then call DVLA on 08702400009 and they will be able to help.

The change of address would be free if you had the paper part to record it on but as you haven't got it you will have to get a form D1 from any post office and apply for a duplicate licence and pay the fee.

You'll need to return your photocard section and apply for a duplicate licence - you can download the relevant form from www.dvla.gov.uk. I would suggest you do it asap because if you ever have to produce your licence i.e. following a non-stop accident that you report or you get caught speeding by Police etc, you will need to produce both parts. Also, the DVLA can fine you up to £1000 if you don't keep your licence details up to date.
