do surface piercings hurt?

Again, it depends where you get them as to how sensitive the part of your body is, but also it depends on the method you choose. Also each person obviously has their own pain tolerance so it's a difficult question to answer.
Getting a surface piercing with a needle doesn't hurt terribly, but the needle cannot replicate the shape of a surface bar, and have a very high tendency to reject or never heal properly. On very rare occasions they last a decent amount of time.
Punch and taper is a more effective but far more painful method. They use the punch to make a large hole straight down and remove a circle of skin, thus it stings terribly, but the taper doesn't hurt at all. Punch and taper provides the correct shape of the correct surface jewelry and so it has about an 85% success rate.
I have both surface hip and surface chest piercings, and the pain was certainly unpleasant, but not unbearable. Moreso than a regular piercing.

It all depends on your pain tolerance and where they are.
I have a nape piercing and I hardly felt it.
