Account hacked?

If someone has hacked into my account and its something like we are sharing the same account, if then I change my password and answer to the secret question so does any such software exists through which that person would get a notification email of my new set password? By notification email I mean similar to the one Yahoo sends to us when our answer gets chosen as the best one. Also is there some way of knowing who the hacker is? And how did my account get hacked in the first place, what did I do wrong? Please someone help me its very important to me.

Update:

I have already changed my passowrd thrice now still I think my account is hacked and every time that I change the password I am suspecting that the hacker figures it out somehow thats why I want to know of any such software that lets the hacker know of the new password that I have set.

    If your account is being bypassed then

    A) Your secret question and answers are known

    B) Your password is known

    C) Your account has a 'backup' email, this is being used to reset your password.

    Its proberly C) and id reccomend reviewing your security settings.

    i believe you would need to change your password. once you do that the hacker couldn't access your account
