What causes anal fistulas?
I've had to lance these things myself because it's too expensive to go to the doctor for it. They never have a solution except hot baths.

Usually they are the result of anal abscesses.They may be caused by appendiceal abscess (as with appendicitis), diverticular abscess (a complication of diverticulitis) or Crohn's disease (an inflammatory disease of the small bowel and rectal area). You may have difficulty treating it yourself. If you see aphysician, it should probably be a general or a colorectal surgeon.
