Its a letter that you will send to apply for a position. This letter should be directed at the company (and not generic). You should state your experiences in work, your skills and strengths and why you would be an excellent candidate for xyz company.

Make sure to keep you handwriting neat and only use fine paper (no lines), most stationary stores will have nice paper. A little tip to keep the sentences straight is to put a heavily lined page behind it.

You may want to type up a 1st draft beforehand and read and re-read when done for gramatical errors (sic). when you're happy jot it down in your finest writing.

Structures vary, but generally you put your name and add on the top right, the date above the intro and (ive seen before) the company's name on top left.

State where you saw the ad placed, let them know you look forward to their response and finish it Yours Sincerely.

Best of luck. Hope you get it.

Modified to add: sorry, you said 'fax' in that case it does not matter a jot what type of paper you use so long as there's no lines. If its postal, go with the good stuff.

A intro letter is when you would introduce yourself. Give some info about why you would like to work there, what you are good at, what got you interested in the position (i.e.maybe it was something in the ad that caught your attention) Market your talents for the position, but don't over due it.

A cover letter is when you would again introduce yourself; tell them you have enclosed your resume for the position and why you would like to be consider for the position. I think the intro letter would contain more information than the cover letter.
