Will I have any problems quitting my job while on unpaid FMLA?
I did not have STD while I was pregnant and just took the 12 weeks unpaid FMLA. I'm supposed to return in 3 weeks, but I have decided to submit a resignation and not return to the job. I used my accrued vacation/sick time to cover the premiums for my insurance, and after that was used I cancelled my benefits. So as of now I have no financial ties to the company at all, I'm just out on the unpaid fmla. Will I be responsible for anything? When should my effective date of resignation be?

No, no problem - and not at all unusual for new moms.
Make the effective date either immediate or the date you were supposed to return. Since you aren't getting any benefits from them, it really doesn't matter.

What I wonder is why you did not receive STD when you were 100% eligible regardless if you took FMLA. You are under no financial obligation to this company to pay back anything since you utilized what was rightfully yours per your companies policy as an employee. It is quite common for women not to return to the position after giving birth for numerous reasons. You can submit a resignation at anytime with no notice since you presently are not at the office nor are you receiving any benefits, etc....
