I haven't played in a while, but I kinda know how...
If I can remember correctly, then you just need to befriend the person at the clothing shop, and he will eventually ask you if you could make some clothes for him to make his inventory a little more exciting.
Sadly, if I can remember correctly, you can't wear the clothes you design.
The designing is like Animal Crossing (if you've played that).
