What determines how the grain in a piece of wood will be formed?
i.e., curly maple vs hard maple? Why did one tree develop a "fancier" grain than another?

The growth habit of the tree and the environmental conditions in which it is grown. Other factors include how the wood is milled (cross cut, flat sawn, quarter sawn, etc.) and such things as infections (burl wood).
