Can anyone suggest a birthday gift idea for a nurse in her late 40's?

A stethoscope if she has a real old one and needs replacing or a pair of brand new white tennis shoes. If none of the above, take her out to a nice dinner or a gift certificate to see a movie.

It does seem to get harder to shop for people the older a person gets. How well do you know her? If she is a friend, just get her something you know she'd like. Otherwise, gift certificates to restaurants or her favorite shopping location would probably be welcome. Or consider if she would like a massage or other spa gift. Consider just taking her out to a movie or lunch as well. I'd stay away from knick knacks or other gifts that are based on her career unless you know she'd appreciate it or needs it.

Is there a "Chicken Soup for the Nurse's Soul" book?

How bout a cake with a cute Dr. in the half nude pic. powder sprayed on the top. Just a thought!

If she believes in God...

then give her a cross necklass, on the card could say, 'God loves you for taking care of patients'.

If she believes in something else...

then give her a gift that represents what she believe in, and repeat that line above, but change 'God' to the thing she believes in.
