You have to become a YouTube partner. You can send in an application to http://www.youtube.com/partner but you need lots of subscribes, views and have non-copyrighted things in your videos.

On YouTube click on your acct button ..
at the very bottom of that page click on "Developer Api's"
then on the left side of that screen go down around 3/4 of the way
and click on "advertising programs."
all the info should be in there .. it sounds like you want a banner.
