When you put website and submit to search engines then it is visible for the world.
.com , .us , .org are just extensions of domains if you want to use .us then ppl take it as that you are targeting usa audience or your business is in USA.
You have to pay for domain and hosting. Now a days many hosts offer free domain with hosting packages. There are many good names like go daddy.com and namecheap.com for domains. And host gator.com , liquidweb.com , webhostingbuzz.com for hosting.
When you will buy hosting you will get free domain name. nO need to pay extra dollars if you are registering .com, .net , .org extensions. But for .us you have to pay extra $.
