Your van is leaking trans fluid, probably from a trans cooler line. If it is still low on fluid, get it to a shop and have it repaired. In the meantime, do not let it too low on trans fluid while driving, that will ruin your trans. I trust you are adding ATF+4 trans fluid.

I'd be checking for any leaks asap , plus making sure that you're using the right transmission fluid that's recommended by your owner's manual , also if you've got over 75,000 miles on it I'd be changing the transmission filter and fluid on it asap too.

Hope that helps and best of luck.By the way keep in mind that part of your rad actually is used to cool the transmission fluid and if you've ever over heated that van it's a good idea to change the transmission fluid when ever this occurs, plus change the oil too asap.Remember that transmissions normally don't use up fluids or go low on fluid unless it's burning it or there is a leak.

One last note check the transmission fluids colour and if it's brownish change it asap , plus if you smell it and it smells like burnt cigars change it asap.Be sure you always have the proper factory recommended transmission filter kits used and fluids.
