Whats wrong with the Ford Ranger?
I have a 1991 Ford Ranger 2.3 with a 5 speed and every now and then, usually just when the engine is cold, it shakes like crazy and barely stays running at idle (Sounds like it is missing almost). The "PIP" senor and EGR was just recently replaced. Let alone it has no power when it is doing the shaking thing and revs up real slow and barely drives. After the engine warms it kicks in with full power and drives like new. Thanks.
Not the drive-shaft, does it when the car is not moving too.

I would check the air and temperature sensor , plus check for any rusty or corroded connections going to at the the sensors , also I'd change the gas filter asap.Keep in mind that even a dirty gas filter can cause such problems bec when sits over night etc it dries up then as you force gas through the system the filter gets wet and allows more gas to flow through it so be sure you do change it and change it yrly after wards.
Due to the fact that's it's a 91 I'd really be wanting to clean the whole charging system from the fuse box , power junctions , battery , starter , alternator , engine block ground one by one with a wire brush etc.
Hope that helps and best of luck.By the way besides changing the gas filter etc I'd run some gas treatment like Lucas through it and once running a few tanks of gas through it change the gas filter again.Don't forget to check the air filter , PCV valve etc too.

Might be an error in the PATS (Passive Anti-theft System). If your key has a little letter S on one side of it, it has a PATS. If you do, try this to determine whether or not that's what it is: go to start the truck and note the "theft" light. If it goes out when you turn the ignition, it should start. If it doesn't, but instead flashes, you need to have it examined. Take it to a local Ford dealer and have them run some codes on it. The key might need to be reprogrammed, or, if that doesn't work, a new key altogether (the chip might be bad). Sorry if that's not the problem!

An obd-2 scanner wont work, as the operating system obd-2 wasn't fully introduced until 96. You can get a scanner for a 91 ford though, but it won't tell you much. The early fuel injection systems were pretty simple, so check your coolant temp sensor/switch, air temp sensor, it sounds like something isn't compensating for the cold motor and trying to run the truck like its already warm.

Check the obvious first. Plugs, wires, cap, rotor, and air filter. If everything is ok there, check the throttle position sensor and the coolant temperature sensor.
