It's usually rape by an older family member of a dependent and vulnerable younger one, but incest is not always unwilling. There are many adult consensual incest couples living secret lives together, and the only pain they feel about it is having to keep the secret to avoid outside interference.

Incest is not criminalized in several countries - Spain, Portugal, France, Belgium, Japan, Sweden in some cases, Italy sans scandal, and Israel, and Romania has considered repealing it's incest law. I say, if no harm is done, it is nobody else's business.
