You can get tb from handling fish that are infected, however this chance is very rare since there are very few fish tb cases ever. Secondly if you have a cut on your hands yes you can get a bacterial ifection from DIRTY tank water. However again very slight risk. you have the same risk as if you have a cut and go and play in the garden. Parasites not likely since water parasites rarely can live out side water.
There are less than 10 cases each year of a human getting something they suspect is from a fish tank.

It's very possible for your other fish to get any bacterial, fungal or parasitic disease from any kind of fish you buy but not yourself Any of these three diseases can also lay hibernated in your tank until it is exposed to a stressful event, and all tanks can be found with any 3 of these types of diseases, it's just natural. I would limit putting my hands in the fish tank whether the water is dirty or not, the type of soap you use can effect your fish, your moisturiser, your perfume, etc. It is ok to do but only do it when neccessary, and if you can, then wear gloves!
Usually when you buy a brand new tank, it would be sterile, second hand ones however, you can't be guaranteed this.
Either way, when you buy a new tank, make sure you clean it with hot water, and clean anything you are putting in there except the fish in cold water.
Aquarium salt is great to have on hand, you can get it at your LFS. It's great for curing diseases and helping fish function better.
You aren't likely to get diseases from your fish tank unless you don't wash your hands! There are very, very rare cases of getting a type of fungal disease from your fish if you are in direct contact with it.

Extremely unlikely.
You are more likely to catch something from natural lakes or rivers than a fish tank.
Although fish do get a disease called TB, it's not the same bacteria that infects humans.
You should still be carefull if you have open wounds etc as any fish tank is not sterile enviroment, and never can be.
