Take her to the vet and the vet can preform all of these for you...
This needs to be done by someone experienced and OTC wormers often contain harsh chemicals that can make your puppy sick...
If you can't afford or can't take her to the vet then take her to a rescue or shelter that can give her the medical attention she needs...This dog is at risk for all kinds of diseases ...keep her inside and away from other dogs....

You don't give her anything. This puppy needs to be taken to the vet for a health exam, the correct shots and a de-wormer from the vet because OTC meds don't work. You'll need to take a stool sample with you so the vet can test for worms.
And please, keep this puppy away from all other dogs because she doesn't have any protection against distemper or parvo right now!

call and ask a vet i think she will have to have 1 set of puppyshots before her adult shots.they will worm her too.

this is not something you should personally give her.
take her to the vet and they will give her what is appropriate.
