If your sister is in Maryland, let her contact the Maryland State of Nursing Board for your convenience and let her explain your situation. That way, you will know exactly what to do and avoid mistakes or misrepresentations. Let her contact:

DECLARATION OF PRIMARY STATE OF RESIDENCE
Primary state of residence is defined as “the state of a person’s declared fixed permanent and principal home for
legal purposes; domicile. Documentation of state of residence includes a valid driver’s license with a home
address, voter registration card with a home address, and/or the state declared as the state of residency on the
last federal tax return.
It is imperative that you inform the Maryland Board as to which scenario best suits your particular situation, to ensure that appropriate procedures are followed in issuing your Maryland license or in directing you to contact the appropriate state(s) to apply for and receive a license.
Please note, if you are in the process of moving to Maryland and declaring Maryland as your state of residence, you must provide the Maryland Board with a Maryland address within 30 days of relocating to this state.
I hope you resolve your dilemma soonest. I hope you will have a good life in Maryland. I remember Baltimore and Annapolis from previous visits. I loved it there! Good luck!

for all intents and purposes, maryland is your home now. use your sister's address. attach a letter explaining that once you get your certification, you might be changing residence (could be in maryland or another state).
read the fine prints of the form. there ought to be a number there that you can call.
