I would, as long as you can find some that are comfortable under them. The point of wearing the spankies is to provide extra coverage, so I'd wear underwear as well. It might feel bulky at first but you will get used to it!

Yes, you do. I'm a cheerleader and we're supposed to wear underwear under our spankies. I wear compression shorts for like all my other sports (hockey, lacrosse, dance) under my uniform or shorts or pants or whatever I'm wearing, and I wear underwear under them.
