how do I replace a concrete patio?

I have a concrete patio that has shifted and cracked in 3 different places. The concrete is about 3 to 4 inches thick. I want to replace it with stones.

I am using an 8 lb sledge to crack the concrete apart. I know using a jack hammer would be faster but I need the therapy.

My question is, what is the best thing to do to prepare the hole before putting down the new stone? I have no ideas here so most anything would help.

Do you have any websites or recommend and specific books for this?

    I built mine from sand stone slabs That were picked miles away. I would first remove all the concrete and level up the location as best I was able then lay the stones like you would a puzzle seeing where they fit best leaving as small a crack between them as possible then fill in the cracks with dirt. This patio is not absolutely level but I don't care.It's a nice place to sit in the shade of some tremendous oak trees!
