A "Cleveland steamer" usually refers to a form of coprophilia, involving a specific sexual fetish act:

Cleveland does have a company that supplies steam for heating and chilled water through a major network of pipes, mostly to older downtown buildings, but I've never heard people talk about it much or refer to it as a "Cleveland steamer."
