i hav a friend who sticks out their tongue frequently, not for fun but in a serious way, is it a disease?

Unfortunately, you do not state whether your friend has, or had a psychiatric or psychotic disorder. I mention this because your description of the sticking out of her tongue is similar to that of a condition called Tardive dyskinesia and is a neurological disorder consisting of abnormal, involuntary body movements caused by certain medicines. It is usually associated with long-term use of medicines for treating schizophrenia and other psychotic disorders. Symptoms of tardive dyskinesia may include - involuntary movements of the face, including frowning, blinking, smiling, lip licking, mouth puckering, biting or chewing, clenching the jaw, sticking out the tongue, or rolling the tongue around in the mouth, involuntary movements of the hands, arms, feet, or legs, such as twitching the hands or tapping the feet, trunk movements, such as rocking, twisting, or squirming, grunting or trouble speaking because of involuntary movements of the diaphragm. The earlier the problem is noticed and treatment begun, the better chance there is that the abnormal movements will go away. Most patients have a noticeable improvement in their symptoms within a year and a half.

Yes I have. My birds don't like my tongue and will bite it if I stick it in their face. Locally there is a buffalo reserve. You can stick your tongue out at the buffalo, but it's not a good idea to piss them off. They can break through the gate and have done it on occassion, wandering onto the public roads. I picture myself lying on the ground with my tongue REALLY sticking out in that scenario.

tics or Tourettes? Maybe even tardive dyskinesia if they've taken psych meds for a longer period of time?

Have you seen a dog doing it in summer.May be it is an animal instinct.It is a part of yoga exercise to beet heat .
