"A-t-il" and "A-t-elle" are the inverted forms of "Il a" (He has) and "Elle a" (She has). These inverted forms are used to turn a statement into a question. There is a "t" inserted in between the 2 words so as to aid pronunciation (since there is no consonant sound between the two words).

Therefore, "A qui a-t-il envoyé un lettre?" means:

"To whom did he send a letter?"
