I recently saw a map which depicted how much debt each country had. Every single major country had some form of billions or trillions of debt. Who is everyone in debt to? In theory if you're in debt, you have no money to loan out, so how is everyone in debt?

the banks have all the money. Cartel-like, they all team up and what not, World bank loans to nations. Its not the people that are in debt, its pretty much the government
