'szigorlat' is a comprehensive exam
'szigorló év' is the last year of university studies in which you will take the final exams
'szigorló orvos' is the medical student in the last university year who will take the final exams at the end of the year to get the medical degree

I'm not a native speaker of Hungarian but I believe it means a candidate for the degree of doctor, a student sitting in for the final university examination, a medical student reading / sitting for the final examination, a candidate for a doctor degree or a beginner physician or intern.
A final medical examination, pretty much.
