In the U.S., Psychiatry Residency is not at all competitive overall, but there are definitely some highly competitive Psychiatry Residency Programs that exist.

Neurology and Cardiology are somewhat more competitive, but not as competitive as some other fields, such as Dermatology and Radiology, for example. To become a Cardiologist, one must enter an Internal Medicine Residency (or a Pediatrics Residency if one wishes to become a Pediatric Cardiologist) and then complete a Cardiology Fellowship. Internal Medicine Residency is about average as far as competitiveness, with some highly prestigious programs that are difficult to match into and some less competitive community programs that are easier to match into. After residency, you must apply for a Cardiology Fellowship position. It can be difficult to obtain a Cardiology Fellowship these days, especially adult Cardiology but it is definitely doable if one obtains good residency training and participates in research (among other things).

Neurology is also middle of the road as far as competitiveness, with some less competitive programs and some highly prestigious programs out there. After graduating from medical school, in order to do Neurology you must complete a one-year intern training year and three years of Neurology. Therefore, there is no fellowship to apply to in order to be a Neurologist. That being said, you can subspecialize in Neurology and complete a fellowship if you so desire, but unlike Cardiology, it is not required.

You have to get your MD before you can specialize in psychiatry, so you have to be a fully fledged doctor before you even start. At least that is my understanding of it.
