My Lhasa Apso makes this horrid sound like he's gonna throw up, but never does, any idea what's wrong?

I've had a couple different answers, one is that it's like a backwards hiccup and another that it's a backwards sneeze.... which one if either is it?

It's completely normal for him, he has done it just about all his life and he's 4 now.

I have two chihuahuas that both do this on occasion and it is normal for the breed. While I don't know if it is normal for Lhasa Apsos, it is not a big deal even though it can seem scary.

You can stop the reverse sneezing by gently massaging your dog's throat, by blowing in their face to force them to breath through their mouths, or startle them/divert their attention elsewhere to help the reverse sneezes to stop.

IF none of those things work to stop the fits, there is always the good ol' vet to have check out the little guy...but I'm sure your dog is fine.

Reverse sneezes are very common as mentioned below, but so are collapsing trachea particularly in brachiocephalic breeds (pushed in noses). Their trachea get irritated easily. These are some of the only breeds that it is recommended that you use a harness on instead of a collar as it removes the pressure. If helping your dog to swallow by gently blowing air in its nose doesn't quickly stop the problem, you should have it reviewed by a vet just to make sure that there isn't something more extensive going on.

If this is just a recent occurence, kennel cough is also a possiblity.

backwards hiccups and backwards sneezing are exactly the same thing. My dog does the same thing and it doesn't ever seem to cause her any additional trouble.
