Medical term: Status Post s/p?

Please explain in plain English what is "Traumatic hydrocephalus s/p V-P shunt replacement on 7/24", Does this mean because of Traumatic hydrocephalus condition, V-P shunt replacement was done to cure/improve the condition on 7/24? Thanks.

    The way it's written, it appears to mean that the traumatic hydrocephalus occurred after the VP shunt was replaced, but that would not be an expected order of events. Being written "Traumatic hydrocephalus. s/p VP shunt replacement on 7/24" would make more sense, in that the shunt is typically used to correct hydrocephalus.
