Does it freeze in the cargo hold of an airplane?

No it does not- its temperature controlled via the planes exhaust (not going to be warm-- its something like 50 degrees down there, but obviously they cant have things come out frozen- too many people put things in luggage that cant get that cold). The people that say it freezes are incorrect.

I've packed loads of medicine in my checkin baggage before when I needed to not look like a druggie going through the airport and was travelling with 7 months worth of medication (about 30 boxes). It didn't come out with ice on it but I always have it in a cloth carry bag and then wrap my clothes around the bag covering all sides of the medicine to protect a bit from people not taking care of my baggage properly. This would also help with keeping it warmer but not hot.

My medication is all tablets so I have never put liquids underneath except Shampoo and that has always came out still in normal liquid form.

If you are not going to look like a druggie going through the airport I would try and take most in my carry on baggage but put some in my checkin baggage. This helps in case someone steals your carry on bag or something like that (which has happened to me once and I had about 2 months worth of medication in my carryon bag). It also helps in not getting looks of what the hell are you carrying in your bag and having to pull it all out in the airport.

You never want to pack medicine in a check bag. Put it in a carry on and keep it with you.

you can advise the airline of the temperature requirements, and they can set the temperature accordingly.
